Physical Examination

Athletes must have a current sports qualifying physical exam on file in the MS/HS office. Physical examination taken April 1 and thereafter is valid for the following two school years; physical examination taken before April 1 is valid only for the remainder of that school year and the following school year. 

Once you have your physical completed by a doctor, you can turn it into the MS/HS office or you can scan it and submit the completed Physical Forms online. 

Football Equipment Fee is at $25.
